Having downloadable forms on the World Wide Web presents certain problems. Depending on the style and layout of the form, it can be difficult to use hypertext mark-up language (HTML) to create a form. Creating a scanned image of the form resulted in extremely large images with marginal quality. Different computer platforms and displays do unusual things to the image. The only way we could find to have a consistent look and style for forms and similar documents was to use the PDF format, which was specifically designed for this purpose. We appreciate your understanding in this matter.
